1. Start with your health plan to confirm eligibility. If needed, they can conference-call SilverSneakers to help retrieve your access code.
2. Call SilverSneakers at 866-584-7389 and use the automated system to check eligibility and, if you’re in a participating health plan, receive your access code.
3. Visit silversneakers.com to check eligibility instantly. Click the blue “Check Eligibility” button and enter your personal information and email.
